i don't see an item page created by your account. did you choose a unique identifier (title)? if you chose one already in use the page would not be created. the system would suggest one (below the title filed) which you could use.

try starting over, click the Upload button. it should show that your field is still uploaded waiting to complete the process. choose a new identifier, see if it accepts it or suggests a different one and try to finish.
